Apprenticeship Levy Transfer Request Part 2: Apprentice(s) Information

Thank you for completing a levy transfer request. As a second step in the process, as soon as you know which individual(s) are undertaking the apprenticeship(s), please also complete the following form to give us more information about them.

Please complete ONE FORM with reference to ONE APPRENTICE and then refresh the page in order to submit additional forms, if you are applying for funding for multiple apprentices. If you have a cohort of more than 5 apprentices, contact us at info@workwhile.org.uk and we can send across a more suitable table.

Why do we ask for this information?

We need this information about your apprentice(s) for the following reasons:

  • To match you with a suitable levy donor, many of whom have a focus on improving equality and diversity within the workforce
  • To evaluate our work and report to our funders, who enable us to provide this support free of charge
  • To send your apprentice(s) one experience survey when they are approximately 6 months into their apprenticeship

Apprentice’s names and contact details will never be shared externally. When shared with potential levy donors, all information is anonymised; when used for evaluation purposes, all data is aggregated. The data we collect is not used for any other purposes beyond those stated here.

GDPR information

If you share this information with us on behalf of apprentice(s) (rather than asking them to complete the form), you must ensure that you have appropriate permission to store and share the information.

All data is held in Salesforce, a secure cloud-based database solution. Access is restricted to Workwhile staff only, with appropriate levels of permissions for their roles. The Workwhile Director controls access to salesforce and the permissions within it.

Data Error and Data Breach

Any errors in data should be reported to your Workwhile Account Manager, or available member of the Workwhile team, in the first instance.

Any known breaches of data, such as accidental loss or damage or unauthorised disclosure, must be reported to your Account Manager or the Workwhile Director immediately. This will be managed according to the privacy policy of IPPR, liaising with the ICO where neccessary and appropriate.
