Please note that by completing and submitting the form:
- you're giving Workwhile, and the programme's evaluators Verian and the Institute of Employment Studies, permission to contact you, your nominated line manager and apprentice/s about the programme and its evaluation activities. For more information about how the data will be used for the evaluation see here.
- you're confirming the eligibility of your organisation and your nominated line manager, in line with the questions below
- you're confirming that you have permission to share contact details for your line manager and apprentice/s
- you're confirming that you understand that your organisation will be allocated at random to either the treatment or control group (for more details about the randomised controlled trial, click here)
- you're committing to support your nominated line manager to complete the programme activities, and them and their apprentice/s to complete short evaluation activities
